Form Description:

Welcome! Please complete this form to provide your consent for your child’s participation in Leyenda Spark, a program of the Leyenda Foundation, held in conjunction with the Leyenda Foundation Convention.

Thank you for supporting your child’s musical journey!

PARTICIPATION CONSENT

I, the undersigned parent/legal guardian, give permission for my child to participate in Leyenda Spark, a program of the Leyenda Foundation. I understand that this program includes participation in:

• Guitar workshops

• Rehearsals and ensemble activities

• Masterclasses

• Public performances (if applicable)

• Related educational activities organized by the Leyenda Foundation during and after the Leyenda Foundation Convention.

I understand that participation is voluntary and that the Leyenda Foundation will take reasonable precautions to ensure the safety and well-being of all participants.

LIABILITY WAIVER & MEDICAL EMERGENCY AUTHORIZATION

I understand that participation in Leyenda Spark involves inherent risks, including physical activity, transportation to and from venues, and participation in public events.

I hereby release and hold harmless the Leyenda Foundation, its officers, directors, employees, agents, and volunteers from any liability, claims, or causes of action arising out of or in connection with participation in Leyenda Spark.

In the event of a medical emergency, I authorize the Leyenda Foundation staff or designated volunteers to obtain necessary emergency medical treatment for my child. I agree to be financially responsible for any costs incurred for such treatment.

I understand that the information in the application is true to the best of my knowledge and that the recording submitted is my own unedited performance. I also understand that Oklahoma City University and the Leyenda Foundation reserves the right to discharge students who have submitted false or misleading information in the application process.

If accepted as a student of any of program of the Leyenda Foundation I agree not to bring any alcoholic beverages or firearms onto the OCU premises or to possess or engage in the illegal use of drugs while a student of the Leyenda Spark.

The applicant consents to the creation and distribution of any recording, broadcast, or electronic transmission of any kind, without compensation, made by Oklahoma City University and the Leyenda Foundation. The applicant similarly consents to the use of his/her likeness in photographs, video or any visual media created and distributed by Oklahoma City University, Leyenda Spark, and the Leyenda Foundation and consents to the creation and distribution of any recording, broadcast, or electronic transmission of any kind, without compensation, made by Oklahoma City University and the Leyenda Foundation. The applicant similarly consents to the use of his/her likeness in photographs, video or any visual media created and distributed by Oklahoma City University, Leyenda Spark, and the Leyenda Foundation.
